Admission Requirements

High School Graduates

• High School Report Card (Form 138)

• Letter of recommendation from the Principal, Guidance Counselor or Office of Student Affairs of school of origin

• Birth Certificate (photocopy)

• Two ID pictures (1X1)

Transferees

• Certified true copy of grades or transcript of records

• Transfer credentials or honorable dismissal

• Letter of recommendation from the Dean, Guidance Counselor or Officer of Student Affairs of school of origin

• Birth Certificate (photocopy)

• Two ID pictures (1x1)

Special Admission Requirements for College Applicants

Married Applicants

• Original copy of marriage contract for presentation and photocopy for submission

Foreign Applicants

• Certification of registration for aliens issued by the Bureau of Immigration

• Birth certificate (with an English translation and duly notarized)

• Passport

  Required documents in securing Student Visa:

  • Letter of acceptance from the admitting school with official dry seal.

  • Notarized proof of adequate financial support.

  • Scholastic records and diploma duly authenticated by the Philippine Embassy or consulate in the applicant's country of origin.

  • Quarantine Medical Clearance from the National Quarantine Office in the Philippines.

  • Five copies (5) of original personal history statement with a 2x2 inch photograph recently taken.

Cross-enrollees

• Written permission to cross-enroll from home school

Special Students

• Letter of request for admission to the Records and Admission Office
